Staff: Staff Forms
On this page, you'll find plenty of items for download. The page is broken into two sections. The first section is where you can download all of the forms and information you'll need to apply to work at Camp Dark Waters. The second section has the forms you'll need after you've accepted a position here.
Application Packet:
Voluntary Disclosure Statement
Personnel Policies and Procedures.
Both the Staff Application and Voluntary Disclosure Statement must be returned to camp in order to be considered for employment. Camp Dark Waters will perform a Background and Reference Check on all persons seeking employment.
HiredStaff:
A completed Health History and Physician's Phyisical Form must be returned to Camp before the beginning of your employment period. Employment cannot begin until both forms are completed and received.
